I/We confirm that we have disclosed any other names that I/We are known by. I/We also confirm that all information I/We provide is accurate, up-to-date, complete, relevant and not misleading. I/we acknowledge that if i/we do not provide all or any part of the information requested on this application form, my/our application for finance may be declined. I/We certify as the Applicant(s) that I/We have read and understood the provisions contained in this Application.

• PRIVACY ACT 2020  AUTHORISATION: By submitting this application form, I/we authorise flywheels for Assessing my/our credit worthiness; Attending to the financing, whether directly or indirectly, of my/our contract(s) and enforcing Flywheels rights thereunder; Marketing and/or informing me/us about the goods and services provided by Flywheels or other associated car dealerships and finance providers. I/we authorise Flywheels to collect from credit reporting agencies, credit providers, my/our employer(s), accountant(s), relatives, or other person(s) such personal, financial and commercial information about me/us for the said purposes. By submitting this application form, I/we authorise flywheels to collect, retain and send details about me/us in this application form to any third party (finance company, credit providers, credit reporting agencies, car dealerships or any other person, in any ordinary course of business) to use the personal information about me/us, for any of the foregoing purposes; of assessing my/our credit worthiness and/or informing me/us about the goods and services provided by flywheels or other associated car dealerships and finance providers.

• Customer Declarations: I/We have individually understood and responded "YES" to the following questions: 1. I/WE can confirm, there will be no foreseeable changes to my/our circumstances that will impact on our ability to make the contracted repayments over the life of the loan contract. If there will be any foreseeable changes, then I/We can confirm I/We will provide those details of the changes in the comments section of this application and confirm that the I/We will still be able to meet the proposed loan repayments despite the change in circumstances. 2. I/We acknowledge that I/We are comfortable with my/our ability to make the repayments under the proposed loan contract and if required are able to adjust my/our discretionary spend to meet the schedule loan repayments. 3. I/we acknowledge that I/we have read and understand the Privacy Declaration and that the personal information I/we provided will be held securely and used for the purpose of applying for finance, with different finance companies. Give permission to conduct  credit enquiry, Driver Licence enquiry, Ministry of Justice enquiry, LTSA enquire and Personal Property Securities Register. 4. I/we acknowledge and agree that the information provided in the application I/we are submitting is true and correct in every respect and that I/we have not withheld any information including expenses that may affect the decision to approve or decline this application for finance.
